Your first visit will include a health history, chiropractic exam, and a detailed adjustment. If you are unable to print the intake form, please let us know so we can arrange to have you fill it out here.
Cancellation Policy
To cancel or reschedule, please contact us 48 hours prior to your appointment. You can send us a text at 805-339-0050 or send us an email : info@harmonywholehealth.com. You can also call the office and leave a message. Cancellations within 24 hours of appointment will be charged a $60 cancellation fee.
